NEW CE Evaluation Process

 All CE evaluation forms are on-line now at www.healthsystemce.org.  

Healthsystemce has been providing NYSCHP with on line CE for years now.  The email address that is in the NYSCHP database is the email address that you will use to sign on to “healthsystemce”.   If you do not remember your password, click on forgot password and you will receive an email of how to reset your password.  

There is a video on the healthsystemce website in the upper right of the homepage that is a 4 minute segment on how to fill in the evaluation form.

A CE code is needed to verify your attendance at the meeting.  The CE code is case sensitive.

Directions to Log-in to NYSCHP Website

To update your contact information:

Go to: www.NYSCHP.org

Click on the "member login" tab on the top of the page. 

Enter your:

·        User name: (FirstnameLastname)

·        Password: NYSCHP  *note: it is CAP sensitive*

Once logged-in, you can edit/view/change your password and update all information.
